Membership Pricing
Direct Path Healthcare is direct primary care that fits your busy life.
Plans are $60 per month per person. DPH offers appointments, prescription refills, and primary care management on your schedule. With affordable, easy-to-use memberships, you know exactly what your membership covers, and the exact price of any additional services. There is a one-time, $40 enrollment fee for new members.

Please note, if you are enrolled in Medicaid, Medicare, or another government healthcare insurance plan, you cannot participate in any direct primary care program.
Membership Services
Direct Path Healthcare monthly memberships include the services listed below.
Annual Wellness Exam
We offer customized Annual Wellness exams for women, men, and children. Exams include a physical assessment, vitals, vision & hearing testing, EKG, body fat analysis, and specific bloodwork.

Health Check-Ups
Your membership includes in-office, telehealth, and home visits for non-emergency services for cold and flu treatment, EKGs, writing prescriptions, and other chronic and acute health issues.

Work, Sports, School Physicals
Whether starting a new job, signing your child up for summer sports, or preparing for back to school, you can use your DPH membership. We offer work, sports, and school physicals - get the documentation you need to work, play, and learn!

Mental Health Screenings
A mental health screening is a standard set of questions to check for signs of a mental disorder. These questions us learn about a person's mood, thinking, behavior, and memory.

Vision & Hearing Screenings
Your membership includes vision and hearing screenings to monitor your health and note changes that might require a specialist or additional medical interventions.
