Meet Tiffany
I'm Tiffany Sullivan, a Family Nurse Practitioner and co-founder of Direct Path Healthcare. I started DPH because I believe everyone deserves a quality, affordable, personalized healthcare experience. After 20+ years working in medicine, I know how frustrating it can be to find a provider you like and get accessible, transparent primary care.

DPH uses a direct primary care model. In the direct primary care model, you and your family pay a monthly fee and get access to wellness visits, mental health screenings, sports/school/and work physicals, on-demand appointment scheduling, in-office visits, and more for a flat monthly fee.

What is Direct Primary Care?
The Direct primary care (DPC) model is a practice and payment model where patients pay the practice directly in the form of a monthly membership that includes a defined set of primary care services. This relationship removes the middleman of insurance, which means no co-pays, no deductibles, no hidden fees, and no surprise medical bills.

For all services and items not included in your monthly membership, we offer clear pricing that explains the service and the cost. We pass our savings directly to our patients because we believe healthcare should be predictable, transparent, and affordable.
